This form is intended to gather information necessary to schedule the student's first lesson.  By completing this form, you acknowledge and agree to the policies and expectations of the Jeffrey Tarr Voice Studio.  I am happy to answer any questions you might have regarding the studio's policies.  

A contract must be signed and tuition must be paid BEFORE enrollment is finalized.
